Community Health Systems Behavioral Health Assess Specialist - LCSW - Night Shift in Antioch, Tennessee

Job Summary

The Behavioral Health Assessment Specialist is responsible for responding to requests for consultative behavioral health assessments by phone and telehealth, including the initial response, the psychiatric triage assessment, the designation of appropriate level of care/disposition, and initiation of placement/referral services. The position works collaboratively to improve the flow of BH patients from ER and inpatient settings into BH programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

Essential Functions

  • Provides comprehensive psychiatric risk assessment of behavioral health patients through telephonic and televideo technology for CHS and non-CHS affiliates to assist in making appropriate level of care decisions.

  • Must be able to formulate diagnostic impressions based upon review of systems, history, symptoms, and current presentation.

  • Responsible and accountable for coordination of continuum of care recommendations through an interdisciplinary approach with the patient, family, requesting provider, hospital staff, and Access Center.

  • Assures EMTALA compliance at all times and reports any related concerns to the BH Access Director immediately.

  • Provides assistance in the development of a Discharge Safety Plan for patients who are discharged using the approved discharge safety form.

  • Uses problem-solving skills to determine the course of action to generate the best patient clinical outcome, the physicians’ preferences, and the transferring and receiving hospitals.

  • Critical thinking skills will be utilized to ensure every patient interaction / assessment promotes the well-being and safety of the patient.

  • Coordinates activities with the BH Access Director to assure accurate record keeping and efficient patient flow.

  • Assists with community education around clinical issues and participates with community events related to Access.

  • Demonstrates knowledge of regulatory requirements such as EMTALA, CHS Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives.

  • Adheres to established policy and procedure and standards of care; escalates issues through the established Chain of Command in a timely manner.

  • When the need arises, will perform other duties as assigned.

  • Additional responsibilities specific to Nurse Practitioners include, but are not limited to the following:

  • As license will allow per state scope of practice: Completion of Psychiatric Consults, Psychiatric Evaluations, History/Physicals, Progress Notes/Rounding, and Psychiatric Discharge Summaries.

  • Completes documentation necessary to adhere to accepted psychiatric and medical standards of care.

  • Provides psychiatric coverage of assigned patients.

  • Provides medication management and participates in the development of individualized treatment plans as needed.

  • Works under the supervision of the Medical Director of Access Center-Behavioral Health

  • Provides superior customer service with all interactions.Ability to organize and prioritize multiple complex tasks.Ability to work effectively with other employees and external parties.

  • Performs other duties as assigned.

  • Complies with all policies and standards.

Qualifications

  • Master's Degree in Social Work, Psychology, Counseling, or related field of study. required

  • Ph.D. Post Masters related coursework preferred

  • 2-4 years direct clinical behavioral healthcare experience required

  • 1-3 years emergency evaluation/consultation experience preferred

Knowledge, Skills and Abilities

  • Knowledge of current behavioral health concepts, principles, and methods of treatments.

  • Knowledge of the behavioral health intake and placement processes, including state and federal standards regarding behavioral health assessment, involuntary petition, and patient rights.

  • Knowledge of payer source implications on discharge plan, resources, and placement options to maximize successful disposition for patients served.

  • Knowledge of the operations of an acute care facility with particular emphasis on the behavioral health patient flow/capacity management/throughput.

  • Demonstrated skills in problem solving, analytical and critical thinking, prioritization, negotiation, conflict resolution, and proactive decision making.

  • Ability to successfully manage multiple tasks simultaneously with a high degree of organization.

  • Ability to establish and maintain collaborative and effective working relationships.

  • Must have excellent written, verbal, and interpersonal communication skills.

  • Work in multiple systems simultaneously

Licenses and Certifications

  • Licensed in the state of residence without restrictions (LCSW, LPC/MHSP, LMFT, CNP,LMSW), with the ability to obtain licensure in additional states required
