Direct Care Provider
We are a 6-bed state license adult foster care home. Located in Bay City MI. We are seeking a full-time day shift and a full-time night shift Direct Care Worker to join our team and work with our patients who have issues completing daily activities due to mental, physical and/or mobility issues.
Shifts are 12hrs from: 9:00 a.m. - 9:00 p.m. & 9:00 p.m. - 9:00 a.m.
Job Responsibilities Include but are not limited to:
- Excellent customer service
- Cuing and redirecting
- Administering medicine and treatments as prescribed by healthcare providers
- Monitoring the physical, mental, and emotional conditions of clients and reporting changes
- Make decisions and resolve issues using critical thinking and analytical skills
- Assisting with personal hygiene needs and performing housekeeping duties
- Maintaining a clean, tidy, and safe environment
- Bathing and dressing of client
- Prepare meals and assist clients with eating and drinking
- The individual must have dependable transportation and be a reliable worker
Knowledge Skills and Abilities:
- Skills in providing services for individuals with special needs with a positive approach
- Ability to work as part of a team
- Must have patience and good communication skills
- Ability to stand, lift and assist with transferring residents
- Problem Solving and react appropriately if an emergency situation arises

Autumn Ridge Gardens has 2 semiprivate bedrooms and 2 private bedrooms. With a spacious open floor plan throughout the facility.
We care for individuals who are male or females, with a licensed capacity of 6 persons served in the home at a time.
We have a physician, a pediatrist, and beautician that provide services in the facility.
We do individual and group outings to promote community involvement and positive outcomes.
Our environment is very clean, and our meals are very tasty.
