Description:
At Saint Francis Hospital, the Stroke Nurse Navigator is part of an interdisciplinary team dedicated to providing comprehensive care to our stroke patients and their families. The Stroke Nurse Navigator utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population to facilitate the continuum of care. The Stroke Nurse Navigator is accountable for facilitation and management of care including providing direct education to patients and families, providing oversight and direction to staff members involved in the care of stroke patients throughout the continuum of their hospital course.
What you will do:
Coordination of post-discharge care, conducting post-discharge outreach, and assisting with stroke education for the staff and the communities we serve
Assists the Stroke Program team with program development and operationalization and works toward achieving the goals set by the Stroke Program
Collaborates with Stroke Coordinator, Stroke Medical Director, Core Stroke Team, and administrative leadership in the development and implementation of Stroke program goals and objectives
Maintains knowledge of best practice and evidence based care regarding neurological and cardiovascular disease entities
Assists with development of stroke care protocols, policies, and procedures
Responds to stroke codes throughout the medical center and initiates coordination of care for the stroke patient
Collaborates with the Care Management team in ensuring proper transition of patient care to rehabilitation facilities and/or community healthcare agencies
Supports the clinical and professional development of nursing and medical staff through mentoring, role modeling, and educating to promote and enhance clinical skills
Assesses patient and family understanding of patient condition, goals of care and ongoing management.
Provides patients and families with education regarding their disease process, medications, treatment and plan of care
Assists with preparing all necessary certification and recertification documents and maintains knowledge of stroke center regulatory guidelines.
As requested, Assists the Stroke Coordinator with data abstraction and statistics.
Minimum Qualifications:
Education : Graduate of an accredited college or university affiliated nursing program. BSN graduate required; MSN preferred
Licensure : Current Licensure as a Registered Nurse RN in Connecticut
Certification : BLS CPR. ACLS or ASLS Preferred
Preferred Experience: 3-5 years’ nursing experience required in a related field such as Acute Care. Neuroscience experience is preferred, but not required.
